If we determine that we are able to assist you, we will provide a diagnosis and discuss treatment options upon examination only. Please mail the following pieces of information to our office:
- Your disc and reports of any X-RAYS, MRI’S and CT scans pertaining to your hip.
- Any additional records pertaining to your hip including Physical Therapy Notes, Operative reports pertaining to the hip, Office Notes or Injection Reports.
*PLEASE BE SURE TO SEND ALL THE INFORMATION TOGETHER. YOUR INFORMATION WILL NOT BE REVIEWED IF THEY ARE RECEIVED SEPARATELY.
